Parent/ Guardian
In consideration of my application and/or minor child’s application and permission to participate in the College Tour, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns. I HEREBY WAIVE AND RELEASE, and forever discharge Parents Connected and its Officers, Directors, Affiliates, Successors and Assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, in law or equity, that may arise from or in any way related to my participation and/or my child’s (“Minor”) participation in any of the functions or activities related to the College Tour trip to various colleges and universities commencing on

with services, lodging, and transportation arranged by Parents Connected. Tour is scheduled to be completed on

Parents Connected provides that this waiver of liability does not apply to any acts of gross negligence or intentional misconduct on its behalf. I understand that participation in any or all of these activities involved in the College Tour trip involves a certain degree of risk that could result in injury or death, property damage or loss of property. I have carefully considered the risk involved and agree to hold Parents Connected, its Officers and Directors, Agents, Successors and Assigns, harmless for all personal injury which could result from participation in these activities. I hereby consent to and authorize the use and reproduction by Parents Connected, or anyone authorized by Parents Connected, of any and all photographs that have been taken of Minor during the course of this College Tour for any purpose, without compensation of any kind. Parents Connected reserves the right to use these photographs in any of its print or electronic publications. I have read, understand and fully agree to the terms of this WAIVER AND RELEASE. I understand and confirm that by signing this WAIVER AND RELEASE I have given up considerable future legal rights. I have signed this Agreement freely, voluntarily, under no duress or threat of duress. I am the parent or legal guardian of the Minor listed below and consent to this Waiver and Release from Liability by my signature below and by executing the attached College Tour Registration, Medical Consent, and Behavior Agreement forms, and I am mentally competent to enter into this Waiver and Release from Liability and Consent Form.
